At the end of the game you are pointed in the direction of a boat that will get you off the island, and given some directions from Juliet. When you get to the dock a boat is heading towards the boat that will get you off the island, but before they can blow up the boat, locke shoots them and blews there boat up.
Then Elliot leaves the island on the ship and as he leaves, he looks up to see the plane he was on come crashing down towards the island and then the next scene is Elliots eye, and he wakes up to see Lisa running towards him asking if he's ok (When she had been killed before).
